The Importance of Support

When our Care Coordination team first reached out to Denise and Tiffany, they were overwhelmed, and irritated with the healthcare system. Denise had no one to help her understand her daughter’s conditions or provide support. So, when our nurse, Keisha, called to introduce herself and tell her about Care Coordination, she worried they were empty promises.

Beginning with her first phone call, Keisha allowed Denise to vent about her problems, and began offering solutions. Keisha also introduced them to Logan, our Community Health Worker.

Denise began completing her weekly observation reports to keep the Care Coordination team up to date on Tiffany’s wellbeing. And the care team began regularly checking in with them both.

Within nine months, Keisha and Logan were able to:

  • Connect them to resources to install a ramp so they can enter and leave the home easily.
  • Educate them on a proper diet, to promote better health and encourage weight loss.
  • Educate them on Sickle Cell disease and symptoms.
  • Encourage regular visits with providers, to stay up to date on condition management.
  • Connect them with a pain clinic and assist with prescription management to control symptoms and pain.
  • Encourage the use of PPE for Tiffany’s immunocompromised condition.
  • Encourage them to visit the local library to check out and read books.
  • Encourage the daughter with a volunteer opportunity at a local homeless shelter.

Tiffany has not been admitted to the hospital in 8 months. She can move easier without as much pain and plans to get a job helping children in the community.

Denise now has someone she can trust and rely on for help. She considers Keisha her “angel” and is no longer overwhelmed with her caregiving responsibilities. She was able to take Tiffany on their first trip in several years, to visit family in NC for the Holidays.

Thanks to the Care Coordination team, Denise and Tiffany are now hopeful about their future and enjoying time together again.
