Enhanced Care Model
Our new Enhanced Care program, HealthyMATCH (More Access to Care at Home to Improve Health), captures insights from caregivers in the home to identify client needs and engage the broader health care system, creating a more holistic approach to home care. The goal of HealthyMATCH is to reduce avoidable events such as emergency room visits and inpatient admissions by increasing preventative care measures for individuals aging in place to improve quality outcomes and lower health care costs.
How it Works
HealthyMATCH integrates insights from caregivers in the home into a holistic picture of health care with the goal of reducing these types of preventable events with preemptive care for individuals in their homes. With the focus on the home as the center of health care, caregivers – the extra set of eyes and ears in the home – are trained to identify and report changes in condition as well as social determinants of health data that triggers interventions that can improve overall client care, driving better health and cost outcomes.
We serve our clients like they are family, using the time we spend with them to create Meaningful Moments that make a difference and ultimately keep them in their homes longer. Our HealthyMATCH program will aim to deliver better health and quality outcomes, which is a win-win for the client, our partners, and the health care system. The foundation of HealthyMATCH is fact-based data from caregivers who spent an average of 17 hours in home each week last year. This data is focused on areas associated with adverse incidents such as, social isolation and loneliness, fall, polypharmacy issues, depression, malnutrition, and other social determinants of health.
Because Help at Home caregivers have better than industry-average retention, they develop close relationships with clients and are well-positioned to detect and report incidents that can help to avoid ER visits, hospitalizations and other unfavorable events. As part of this home care value-based model, these reports trigger insights and outreach by Help at Home nurses and social workers, who coordinate with health care professionals and community resources. This creates an integrated care approach with caregivers serving as the lifeline between clients and the broader health care system.
Caregivers are trained to identify social determinants of health to prevent adverse incidents like; social isolation/loneliness, fall risks, polypharmacy, malnutrition, etc.
