ID/DD Waiver Services - COMING SOON
In-Home Respite is available to individuals with IDD living in a family home and provides temporary, periodic relief to those normally providing care for the eligible individual. Staff provides all the necessary care the usual caregiver would provide during the same time period.
Shared Supported Living
Shared Supported Living services are for individuals age 18 or older who have IDD and are provided in residences either owned or leased by the person or the provider. Direct supervision is not required, but awake staff is available as needed 24 hours per day in order to provide supports for people to live as independently as possible.
Host Homes are private homes where an individual lives with a family and receives personal care and supportive services. The principal caregiver assumes the direct responsibility for the participant's physical, social, and emotional well-being and growth in a family environment.
Community Respite is provided in a community setting designed to provide caregivers an avenue of receiving respite from constant caregiving and provide the person with a place to go which has scheduled activities to address individual preferences/requirements.
Crisis Intervention provides immediate therapeutic intervention, available to an individual on a 24-hour basis, to address personal, social, and/or behavioral problems which are likely to threaten the health and safety of the person or others and may result in the person's removal from his/her current living situation.
Behavior Support provides systematic behavior assessment, Behavior Support Plan development, consultation, restructuring of the environment and training for people with IDD whose maladaptive behaviors are significantly disrupting their progress in learning, self-direction or community participation.
